Your pet will be undergoing general anesthesia plus a surgical procedure.

I understand there is an inherent risk to surgery and anesthesia.  I authorize Columbus Veterinary Clinic to perform surgery and any other procedures deemed necessary and reasonable from any unforseen conditions or complications that may arise during the procedure.
